For the latest help see <a href=\"https:\/\/oihelp.corporate.ifs.com\/help\/p2-server\/entries\/forms\/\">Forms<\/a>.<\/p>\n<p class=\"intro-text\">A Form defines both the type of entries that will be created, and the fields that the entries will contain.<\/p>\n<p class=\"intro-text\">Forms are configured by administrators in Server Management and are used in A-Plus, Case Management and Shift Log.<\/p>\n<p class=\"left-bar\">Related: <a href=\"https:\/\/oihelp.corporate.ifs.com\/help\/p2-server\/entries\/fields\/\">Fields<\/a>, <a href=\"https:\/\/oihelp.corporate.ifs.com\/help\/p2-explorer\/using-entries\/entry-form\/\">Entry Dialog<\/a><\/p>\n<hr \/>\n<h2 class=\"page-subheading\"><span id=\"What_Type_of_Entry_Should_I_Configure\">Types of Forms<\/span><\/h2>\n<p class=\"intro-text\">The different types of forms are designed for different purposes and have different default and mandatory fields. All form types can have any number of fields in addition to the default fields.<\/p>\n<p class=\"intro-text\">There is only one copy of each form, and of all the entries that have been added using the form. Enter a <strong>Short Name<\/strong> that will be used to identify the form in the entry dialog.<\/p>\n<p class=\"intro-text\">6. When configuring a Case Form, you need to select the <strong>Categories<\/strong> the form belongs to. Next, choose the <strong>Fields<\/strong> that you want to appear in the entries that use this form.<\/p>\n<p class=\"intro-text\">The available fields are listed on the left, and the fields contained in the form are listed on the right. On the <strong>General Defaults<\/strong> tab, for each field you can specify:<\/p>\n<ul class=\"intro-text\">\n<li><strong>Editable<\/strong> - users will be able to edit this field in the entry form dialog<\/li>\n<li><strong>Mandatory<\/strong> - this will be a required field in the entry form dialog<\/li>\n<li><strong>Default Value<\/strong> - what, if any, the default value should be for the field.<\/li>\n<\/ul>\n<p class=\"note\"><strong>Caution:<\/strong> Mandatory fields must also be either Editable, have a Default Value or both.<\/p>\n<p class=\"intro-text\">B. Click the <strong>Table View<\/strong> tab to specify:<\/p>\n<ul class=\"intro-text\">\n<li><strong>Hidden<\/strong> - the field will not be shown in the entries table<\/li>\n<li><strong>Width<\/strong> (px) - you can optionally specify a default width for the column of this field in the entries table. If the column is blank, a default width will be calculated<\/li>\n<li><strong>Sort order<\/strong> - click and hold each \"card\" to use drag and drop to change the sort order<\/li>\n<\/ul>\n<p class=\"left-bar\">Related: <a href=\"https:\/\/oihelp.corporate.ifs.com\/help\/p2-explorer\/using-entries\/entry-form\/\">Entry Dialog<\/a>, <a href=\"https:\/\/oihelp.corporate.ifs.com\/help\/p2-explorer\/using-entries\/entries-table\/\">Entries Table<\/a>, <a href=\"https:\/\/oihelp.corporate.ifs.com\/help\/shift-log\/primers\/workbooks\/\">Workbooks<\/a><\/p>\n<p class=\"note\"><strong>Note:<\/strong> The order of the fields in the Table View and Dialog View is not related to the order of the fields in the assign fields box.<\/p>\n<p class=\"intro-text\">C. Click the <strong>Dialog View<\/strong> tab to specify:<\/p>\n<ul class=\"intro-text\">\n<li><strong>Hidden<\/strong> - the field will not be shown in the entry form dialog, which means that users will not be able to change the field's value. Note if you hide a field, you need to specify a default value if the field is deemed mandatory<\/li>\n<li>Depending on the type of field, other options may be available, such as the maximum number of characters for a single line text field, the height for a multiline text field, or Hide Formatting Toolbar<\/li>\n<li><a href=\"https:\/\/oihelp.corporate.ifs.com\/help\/p2-server\/entries\/forms\/#Conditional_Fields\"><strong>Only show when<\/strong><\/a> - set the conditions for hiding or displaying the field in the dialog. The field is displayed when the value of the list field equals or does not equal, the predetermined value.<\/li>\n<li><strong>Sort order<\/strong> - click and hold each \"card\" to use drag and drop to change the sort order<\/li>\n<\/ul>\n<p class=\"left-bar-yellow\"><strong>Tip<\/strong>: System Fields cannot be removed from a Form, but can be hidden from view in the table and dialog.\u00a0<\/p>\n<p class=\"intro-text\">D. Select the\u00a0<strong>Instructions<\/strong> tab to add information about a field. Fields that have added instructions are indicated by the clickable info <img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-55868\" src=\"https:\/\/oihelp.corporate.ifs.com\/help\/wp-content\/uploads\/2022\/06\/Snag_4122bcba.png\" alt=\"\" width=\"9\" height=\"20\" \/> icon to the right of the field in the dialog view.<\/p>\n<p class=\"note\"><strong>Note:<\/strong> Instructions are only viewable in the Shift Log dialog.<\/p>\n<p class=\"intro-text\">E.
